Logan Health is seeking a compassionate, flexible Physical Therapist to join our team in Chester, Montana on a PRN basis. This position is a great opportunity to provide meaningful, individualized care while supporting patients in a close-knit rural community.

Position Responsibilities
Develop, implement, and revise individualized, goal-directed physical therapy plans of care through skilled evaluation and ongoing assessment
Perform comprehensive patient assessments reflecting functional, physical, mental, emotional, psychosocial, and environmental status
Develop and maintain updated individualized care plans that include goals, interventions, medications, and visit frequency orders
Plan, communicate, supervise, and delegate therapy services in a competent and efficient manner
Provide treatment according to established physical therapy standards and department policies and procedures
Actively participate in the rehabilitation process, including admission and discharge evaluations, daily assessments, and individualized goal-setting
Demonstrate excellent customer service and communication with internal and external customers
Complete all required documentation in a timely manner according to payer guidelines and Montana physical therapy practice guidelines
Support rehabilitation services through participation in departmental, facility, and community programs or projects
Achieve productivity standards according to department protocol
Maintain regular and consistent attendance as scheduled by department leadership
Qualifications
Minimum of one (1) of the following required:
Current Montana Physical Therapy license
OR
Graduate of an accredited Physical Therapy program and must successfully obtain Montana Physical Therapy license within thirty (30) days of hire
Additional requirements:
Current HS AED CPR
Excellent organizational skills and attention to detail
Self-starter with strong critical thinking skills and the ability to prioritize effectively
Ability to work both independently and as part of a team
Commitment to maintaining confidentiality
Excellent verbal and written communication skills, including the ability to communicate effectively with a variety of audiences
Strong interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy
Working knowledge of Microsoft Office Suite and the ability to learn additional software as needed
