What You’ll Do:

  Travel to client homes and provide physical therapy services

  Develop functional goals and personalized recovery plans

  Coach patients and caregivers on exercises and modifications

  Communicate with nurses, case managers, and primary care providers

Requirements:

• 
  MN Physical Therapy license in good standing
• 
  Strong interpersonal and adaptability skills
• 
  Valid driver’s license and reliable transportation
