Schedule: Monday-Friday 8:15-5 pm No On Call

Agency: The Christ Hospital Home Health Care

Territory: East End, California, Mt. Washington, Anderson/Newtown, Withamsville, Cherry Grove, PURPLE, Amelia, Batavia, Bethel, Chilo, Felicity, Georgetown, Hamersville, Moscow, Mt. Orab, New Richmond, Newtonsville, Owensville, Ripley, Russellville (West of 62 only), Sardinia, Williamsburg.

The Physical Therapist Assistant (PTA) provides skilled therapy services under the direction of a licensed Physical Therapist within an assigned geographic territory. The PTA educates patients and their families while ensuring patient safety at all times. This role is responsible for accurate, timely documentation and full compliance with all applicable laws and regulations. The company follows the physical therapy scope of practice as defined by the state for licensed PTA personnel.

Qualifications & Attributes

  • Current Physical Therapist Assistant license in the state of employment
  • Minimum of one year of PTA experience in an acute care setting (required)
  • Home health experience (preferred)
  • Strong communication skills with patients, families, physicians, and colleagues
  • Knowledge of therapy best practices
  • Sound clinical judgment and decision-making skills
  • Detail-oriented, observant, and organized
  • Strong work ethic with the ability to prioritize and meet deadlines
  • Valid driver’s license and active auto insurance (must be listed as a driver)

Major Areas of Responsibility

  • Provide skilled therapy services in patients’ homes based on the Physical Therapist’s plan of care
  • Monitor, document, and report patient status, reactions, and progress
  • Perform prescribed physical therapy procedures and treatments
  • Help relieve pain and restore function using therapeutic techniques (exercise, massage, heat, water, etc.)
  • Collaborate with the Physical Therapist to assess needs and achieve functional goals
  • Communicate effectively with physicians, therapists, and team members
  • Participate in case conferences to ensure coordinated patient care
  • Recommend or adjust prosthetic and orthotic devices as appropriate
  • Educate patients, families, and staff on therapy techniques, safety, and equipment use
  • Promote patient independence and self-care
  • Complete all clinical documentation in accordance with company and regulatory standards
  • Participate in audits, training, meetings, and onboarding of new staff
  • Maintain communication with scheduling staff regarding any changes

Essential Functions

  • Travel to patient homes for scheduled visits
  • Maintain consistent attendance at assigned visits
  • Complete documentation within required timeframes:
    • Routine visits: within 24 hours
    • Start of care visits: within 48 hours
  • Follow the established plan of care within PTA scope of practice
  • Complete and submit all required documentation per company standards

Note: Per-visit compensation includes all associated tasks such as preparation, travel, documentation, communication, and coordination of care.

Health Qualifications

  • Must meet all physical job requirements
  • Proof of negative TB test (or completion upon hire); annual testing required thereafter
  • Must be free from conditions that limit ability to perform essential duties
  • May be exposed to bloodborne pathogens
