Join Our Rehabilitation Team

Pleasant Hill Post Acute is seeking a compassionate, motivated, and dependable Physical Therapist Assistant (PTA)to join our rehabilitation team on a PRN basis

The PTA works underthe direction of the supervising Physical Therapist to provide high-quality rehabilitation services that promote mobility, independence, safety, and overall quality of lifefor our residents.

Position Responsibilities

  • Provide skilled physical therapy treatments and interventions as directed by the supervising Physical Therapist.

  • Assist residents in achieving individualizedmobility and functional goals.

  • Complete timely, accurate, and compliant clinical documentation.

  • Maintain a safe and clean treatment environment while following infection control and safety protocols.

  • Communicate regularly with the supervising Physical Therapist regarding resident progress, goals, changes in condition, discharge planning, and care coordination.

  • Collaborate with nursing and other members of the interdisciplinary team.

  • Participate in Resident Care Conferences and rehabilitation meetings as needed.

  • Assist with training and support ofRestorative Aides.

  • Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety techniques, fall prevention, and maintenance programs.

  • Assist with recommendationsfor durable medical equipment and adaptive equipment to promote resident safety and independence.

  • Provide staff educationand in-service training as needed.

  • Report equipment concerns and help ensure rehabilitation equipment remains safe and functional.

  • Participatein quality assurance and performance improvement activities.

  • Maintain strict confidentiality of resident and company information.

  • Develop positive and professional relationships with residents, families, coworkers, and healthcare professionals.

Qualifications

  • Associate Degree from an accredited Physical Therapist Assistant (PTA) program required.

  • Current California Physical Therapist Assistant license required.

  • Skilled nursing or post-acute rehabilitation experience preferred.

  • Strong communication, organization, and teamwork skills.

  • Dependable andflexible with the ability to work on an as-needed basis.

  • Commitment to providing compassionate, resident-centered care.
