Concord Post Acute is seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team on a PRN/per-diem basis. Position Details

  • Position: Physical Therapist Assistant (PTA)

  • Employment Type: PRN / Per-Diem

  • Location: Concord, CA

  • Compensation: Starting at $50–$53/hour (DOE)

  • Department: In-House Rehabilitation Team

Key Responsibilities

  • Provide skilled physical therapy treatments under the direction of the supervising Physical Therapist

  • Accurately document daily treatments, progress notes, and patient outcomes

  • Collaborate with nursing staff, therapists, and interdisciplinary team members regarding resident care plans

  • Participate in patient care conferences and ongoing treatment planning

  • Assist in training and supporting Restorative Aides to promote continuity of care

  • Educate patients, families, and caregivers on maintenance programs and safe discharge planning

  • Maintain professional, compassionate communication with residents, families, and staff

Qualifications

  • Active California Physical Therapist Assistant (PTA) license in good standing
