You'll be out in the community managing a field-based schedule and providing one-on-one Physical Therapy treatments in patients' homes. Your Territory: Western Metro & Buffalo Area We're hiring a PTA to serve patients throughout the Western Twin Cities Metro and communities extending toward Buffalo and surrounding areas . Your territory may include: Cokato, Corcoran, Delano, Greenfield, Hamel, Howard Lake, Independence, Long Lake, Loretto, Maple Plain, Medina, Montrose, Mound, Rockford, Waverly, Watertown, Hanover, Annandale, Kimball, Maple Lake, South Haven, Buffalo, and Cold Spring. Scheduling is coordinated geographically whenever possible to help cluster patient visits and create a more efficient field schedule. What You'll Actually Be Helping Patients Accomplish Your patients aren't coming to you because they want to become better at therapy exercises. They want their lives back. They want to: Walk safely through their home Get out of bed independently Stand up from their favorite chair Navigate stairs Improve their balance Reduce their risk of falling Regain strength following hospitalization or surgery Safely use a walker or other assistive device Get back outside Rely less on family and caregivers Your treatments are what help turn those goals into progress. What You'll Do Working under the direction of the supervising Physical Therapist, you'll: Provide one-on-one Physical Therapy treatments in patients' homes Carry out individualized plans of care established by the Physical Therapist Provide therapeutic exercise and strengthening Perform gait, balance, transfer, and mobility training Help improve endurance, coordination, and functional independence Reinforce home exercise programs Educate patients and caregivers on safe mobility and therapeutic techniques Monitor patient response to treatment Recognize and communicate changes in condition Provide progress updates to the supervising Physical Therapist Collaborate with nursing, OT, SLP, physicians, and interdisciplinary clinicians Complete timely and accurate clinical documentation The Right PTA Can Thrive in a Per-Visit Model Home health rewards a different kind of therapist.
