Home Health Physical Therapist Assistant (PTA)
We are seeking a motivated and patient-focused Physical Therapist Assistant (PTA) to join our Home Health team. This role allows you to provide one-on-one patient care in the home setting while working under the supervision of a Physical Therapist and supported by an experienced clinical leadership team.
Compensation
Salary: $68,000 – $75,000 annually + Bonuses
Predictable full-time schedule
Minimum productivity expectation: 6 points per week

Responsibilities
Provide physical therapy treatments in patients’ homes under PT supervision
Implement treatment plans and therapeutic exercises
Assist patients in improving mobility, strength, balance, and safety
Educate patients and caregivers on home exercise programs
Document visits accurately and timely
Communicate patient progress to supervising Physical Therapist
Ensure compliance with Medicare and home health documentation requirements
Qualifications
Active Physical Therapist Assistant (PTA) license
Home Health experience preferred (not required)
Valid driver’s license and reliable transportation
Strong communication and time-management skills
Comfortable working independently in the field
