San Francisco Post Acute is seeking a compassionate, motivated, and patient-focused Physical Therapist Assistant (PTA) to join our rehabilitation team. This is a great opportunity for an experienced PTA or a new graduate looking to grow their career in a supportive post-acute environment.

Position Overview

As a Physical Therapist Assistant, you will provide skilled therapy services under the direction and supervision of a licensed Physical Therapist. You will play an important role in helping residents improve mobility, strength, balance, functional abilities, and overall independence.

Responsibilities

  • Provide physical therapy treatments under the direction of a licensed Physical Therapist.
  • Assist residents with improving mobility, strength, balance, coordination, and functional independence.
  • Follow individualized treatment plans and therapeutic interventions.
  • Monitor resident response to treatment and communicate progress or concerns to the Physical Therapist.
  • Accurately document treatments, resident progress, and other required clinical information.
  • Collaborate with the interdisciplinary care team to support resident-centered goals.
  • Educate and encourage residents throughout the rehabilitation process.
  • Maintain compliance with facility policies, regulatory requirements, and documentation standards.

Qualifications

  • Current and valid California Physical Therapist Assistant (PTA) license required.
  • Skilled nursing or post-acute care experience preferred.
  • New graduates are welcome to apply!
  • Strong communication and interpersonal skills.

  • Ability to work effectively both independently and as part of a team.
