Job Description

Fellowship Village is seeking a skilled and compassionate Physical Therapist Assistant to join our team on a per diem basis. In this role, you will implement individualized physical therapy programs, assist in rehabilitation efforts, and provide high-quality physical therapy treatments to customers under the supervision of a Licensed Physical Therapist. Your contributions will directly impact the quality of life of our clients, fostering their independence and promoting overall wellness.

Key Responsibilities:

• Implement customer-specific treatment plans developed by the primary Physical Therapist.
• Assist the Physical Therapist in performing evaluations, tests, and complex procedures.
• Guide clients through mobility activities, ensuring they understand the skills and techniques needed for success.
• Collaborate with the therapy team to develop tailored programs that match the needs and capabilities of each client.
• Complete all clinical documentation in a timely and organized manner for easy access in resident charts.
• Actively participate in care meetings and contribute to customer care under the guidance of the Director of Rehabilitation or Office Manager.
• Clean, maintain, and report any necessary repairs for therapy equipment.
• Provide exceptional customer service, ensuring clients and their families experience compassionate, high-quality care.
• Maintain consistent communication with the supervising Physical Therapist regarding client progress and care requirements.

Qualifications:

• Graduate from an accredited Physical Therapist Assistant Program, certified by the American Physical Therapy Association.
• Must hold a valid Physical Therapist Assistant license as required by the state.
• Excellent interpersonal skills, with a strong commitment to providing patient-centered care.
• Ability to work effectively as part of a multidisciplinary team.
