Do they have enough strength to get out of their chair? Are they actually doing the exercises that will keep their recovery moving forward? As a Physical Therapist Assistant with Home Health Care, Inc. , you'll work with patients during one of the most important parts of their recovery: What happens after they get home. Help Make Home a Place They Can Stay For many of our patients, independence can be fragile. A hospitalization, surgery, fall, illness, or extended rehabilitation stay may have changed what they can safely do. Your job is to help them rebuild. Working under the direction of a Physical Therapist, you'll provide skilled one-on-one treatment focused on the mobility, strength, balance, endurance, and functional skills patients need to safely live their everyday lives. Because getting discharged home is one milestone. Being able to remain there safely is an even bigger one. A Focused Northeast Minneapolis & North Metro Territory You'll provide home-based Physical Therapy throughout a concentrated area including: Northeast Minneapolis Columbia Heights New Brighton Hilltop Mounds View 55421 and surrounding communities Patient scheduling is coordinated geographically whenever possible to help cluster visits and create a more efficient field schedule. That means your workday isn't about covering the entire Twin Cities. It's about serving patients within your assigned area and making every visit count. Your Visits Can Change What Happens Next Home health Physical Therapy can be the difference between a patient continuing to regain independence and beginning to lose ground. Your treatment may help someone: Rebuild strength following hospitalization Improve balance after a fall Safely navigate their home Increase walking distance and endurance Improve transfers from beds, chairs, and other surfaces Become more confident using a walker or other assistive device Safely manage stairs Reduce their risk of future falls Become less dependent on a spouse or caregiver Continue progressing toward greater independence You're not just helping someone complete today's treatment. You're helping prepare them for everything they need to do between visits. What You'll Do Working from the plan of care established by the supervising Physical Therapist, you'll: Provide one-on-one Physical Therapy treatments in patients' homes Carry out individualized plans of care Provide therapeutic exercise and strengthening Perform gait and mobility training Work on balance, coordination, transfers, and endurance Reinforce home exercise programs Educate patients and caregivers on safe mobility Help patients develop safer habits within their home environment Monitor response and tolerance to treatment Recognize changes in function or condition Communicate patient progress and concerns to the supervising PT Collaborate with nurses, OTs, SLPs, physicians, and interdisciplinary clinicians Complete accurate and timely clinical documentation You See What Happens Between the Hospital and Independence A clinic can show you how someone walks down a controlled hallway. Home health shows you whether they can get from their bedroom to their kitchen. You'll see the actual stairs. The actual furniture. The throw rug that's becoming a fall risk. The walker sitting across the room when it should be within reach. The family member who's unsure how much assistance to provide.
