Position Title: Physical Therapist
Location: Fremont, CA
Compensation: $56-$61/hour
Schedule: Monday-Friday, 35 hours/week, no weekend rotation
Setting: Skilled Nursing Facility (SNF) / Post-Acute Rehabilitation Center / Nursing Home

Responsibilities
• Provide physical therapy evaluations and interventions for patients
• Develop and implement individualized treatment plans
• Document patient progress and coordinate with interdisciplinary staff
Requirements
• Current Physical Therapist license
• Experience in SNF or rehabilitation settings preferred
• Strong patient care and communication skills
