Pay: $70.00 - $75.00 per hour
Schedule: Full-Time, Part-Time, or Per Diem (PRN)

Responsibilities

As a Physical Therapist, you will:

  • Evaluate residents and develop individualized treatment plans
  • Provide skilled physical therapy services in a skilled nursing and post-acute setting
  • Improve patient mobility, strength, balance, endurance, and functional independence
  • Monitor patient progress and modify treatment plans as appropriate
  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals
  • Educate patients and families regarding treatment goals, recovery expectations, and home exercise programs
  • Complete accurate and timely clinical documentation in compliance with state and federal regulations
  • Contribute to positive patient outcomes and quality-of-care initiatives

Qualifications

  • Current and unrestricted California Physical Therapist License required
  • Graduate of an accredited Physical Therapy program
  • Skilled Nursing Facility (SNF), rehabilitation, long-term care, or post-acute experience preferred
  • Strong clinical assessment and treatment skills
  • Excellent communication, organization, and time-management abilities
  • Ability to work independently while contributing to a team-focused environment
  • Commitment to delivering patient-centered care
