Physical Therapist Responsibilities

  • Evaluate residents and develop individualized treatment plans
  • Provide evidence-based physical therapy services to improve mobility, strength, balance, and functional independence
  • Assess patient needs and establish measurable treatment goals
  • Monitor patient progress and modify treatment plans as appropriate
  • Collaborate with physicians, nursing staff, therapy team members, and families
  • Educate patients and caregivers regarding treatment plans, exercises, and safety recommendations
  • Participate in interdisciplinary care planning meetings and patient conferences
  • Supervise Physical Therapist Assistants (PTAs) as appropriate
  • Recommend adaptive equipment and mobility aids when necessary
  • Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines

Qualifications

  • Current California Physical Therapist (PT) License required
  • Graduate of an accredited Physical Therapy program
  • Strong clinical assessment and treatment planning skills
  • Excellent communication and interpersonal abilities
  • Ability to work effectively within an interdisciplinary healthcare team
  • Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
  • Commitment to delivering compassionate, patient-centered care
