San Bruno Skilled Nursing is seeking a compassionate and motivated Physical Therapist (PT) to join our full-time, in-house rehabilitation team.

We are committed to providing high-quality, resident-centered therapy that helps individuals improve mobility, regain independence, and achieve meaningful functional goals. Whether you're an experienced clinician or a recent graduate, you'll have the opportunity to work in a collaborative environment with support, mentorship, and a strong focus on resident outcomes.

Position Responsibilities

  • Evaluate residents and develop individualized physical therapy treatment plans.

  • Provide skilled physical therapy services to improve mobility, strength, balance, coordination, and functional independence.

  • Establish measurable treatment goals based on each resident’s needs and abilities.

  • Monitor resident progress and adjust treatment plans as appropriate.

  • Document evaluations, treatment sessions, progress, and discharge recommendations accurately and timely.

  • Collaborate with nursing, occupational therapy, speech therapy, physicians, and other members of the interdisciplinary team.

  • Educate residents, families, and caregivers regarding therapy programs, safety, mobility, and home exercise programs.

  • Supervise and provide clinical guidance to Physical Therapist Assistants (PTAs), as applicable.

  • Promote resident safety, dignity, independence, and quality of life.

Qualifications

  • Current and active California Physical Therapist (PT) license required.

  • Strong clinical assessment, treatment, communication, and documentation skills.

  • Ability to work effectively as part of an interdisciplinary team.

  • Compassionate, professional, and resident-centered approach.

  • Passion for geriatric rehabilitation and improving functional outcomes.
