Pay: $65.00 - $75.00 per hour Schedule: Full-Time, Part-Time, or Per Diem (PRN)

Responsibilities

  • Evaluate patients and develop individualized treatment plans.
  • Provide skilled physical therapy services to post-acute and skilled nursing residents.
  • Improve strength, mobility, balance, endurance, and functional independence.
  • Monitor patient progress and adjust treatment plans as needed.
  • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.
  • Educate patients and families on treatment goals and home exercise programs.
  • Maintain accurate, timely, and compliant clinical documentation.

Qualifications

  • Current and unrestricted California Physical Therapist (PT) License required.
  • Previous experience in a skilled nursing facility, rehabilitation, or post-acute setting preferred.
  • Strong clinical, communication, and time-management skills.
  • Ability to work independently while contributing to a team-oriented culture.
  • Passion for delivering exceptional patient-centered care.
