About the Role
We’re looking for a compassionate and skilled Physical Therapist to join our growing home health team. In this role, you’ll have the opportunity to make a meaningful impact by helping patients regain independence and improve their quality of life—to all from where they reside.

What You’ll Do
• Provide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and discharges
• Develop and implement personalized treatment plans based on patient needs and physician orders
• Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement
• Educate patients and caregivers on safety, fall prevention, and daily living strategies
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes
• Assess vital signs as needed and communicate any concerns to the care team
• Recommend and educate on the use of assistive devices and durable medical equipment (DME)
• Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians
• Complete timely and accurate documentation
• Support discharge planning and ensure continuity of care

What We’re Looking For
• Licensed Physical Therapist in the state of Massachusetts
• Degree from an accredited Physical Therapy program
• CPR certification (current)
• Valid driver’s license and reliable transportation
• Home health experience required
• Strong communication, organization, and clinical decision-making skills

Preferred
• At least 1 year of clinical experience

Work Environment & Physical Requirements
This role involves travel to patient homes and requires physical activity including standing, walking, bending, and occasional lifting or assisting patients. Exposure to infectious materials may occur; all safety protocols are strictly followed.
