Job Description


A Physical Therapist is an individual licensed to perform examination, treatment or instructions to detect, assess, prevent, correct and alleviate physical disability and pain from injury, disease, disorders or physical deformities through the use of specific evaluation technique, massage or manipulation, therapeutic exercises, hydrotherapy, and various forms of energy. The Physical Therapist is responsible for the delivery of therapy services to patients in accordance with an established physician plan of care and in compliance with applicable licensure/certification
requirements, the Texas State Practice Act, and Hunt Memorial Hospital District policies and procedures. The position summary is derived from and complies with the Texas Physical Therapy Practice Act and the rules of the Texas State Board of Physical Therapy Examiners (2001).
POSITION REQUIREMENTS
Minimum Education:
Graduate of an approved physical therapy program with evidence of current licensure in the state of Texas.
Maintain continuing education requirements as defined by the Texas State Board of Physical Therapy Examiners.
Minimum Work Experience: None
Required Licenses/Certifications
Evidence of current licensure in the state of Texas. CPR certified.
Required Skills, Knowledge, and Abilities
Effective verbal and written communication skills. Ability to meet minimum hiring standards.

JOB SPECIFIC FUNCTIONS
1. Demonstrates an understanding of and adherence to the HMHD Compliance Plan.
2. Conduct reflects HMHD's values and a commitment to HMHD's Code of Conduct.
3. Attends the required corporate integrity and compliance training and education programs.
4. Demonstrates proficiency in understanding the materials presented during the corporate integrity and compliance training and education program.
5. Complies with all HIPAA standards.
6. Performs evaluations, develops and implements the Physical Therapy Plan of Treatment in conjunction with the referring and/or treating physician, patient, and family.
7. Evaluates the patient within 72 hours.
8. Performs initial patient evaluation and plans appropriate therapy program with approval of the attending physician.
9. Initiates the plan of care and makes necessary changes as patient condition dictates and in accordance with physician's approval.
10. Plans appropriate STG's, LTG's and documents progress in attainment of same.
11. Treats patients for the purpose of relieving pain, developing/restoring functional capability, and maintaining maximum performance.
12. Directs and aides patients in active and passive exercise, muscle re-education, and functional training activities of daily living, such as transfers and gait training.
13. Seeks appropriate medical advice as necessary.
14. Maintains and submits all required patient documentation in a timely manner.
15. Verifies physician's orders prior to initiating treatment.
16. Completes initial evaluation.
17. Completes daily progress notes.
18. Documents teaching and patient/family response.
19. Completes discharge summaries.
20. Participates in discharge planning.
21. Confers with physicians and staff as necessary; documents all pertinent information and recommendations.
22. Orients, instructs, and directs work activities of assistants and aides
23. Develops and instructs assistants in the established plan of care and any ongoing changes.
24. Provides complete instruction for assistants and/or aides performing procedures.
25. Supervises care rendered by assistants and aides.
26. Plans and conducts lectures and training programs on physical therapy and related topics for Rehabilitation Services staff, medical staff, clinical departments, students and community groups.
27. Attends ongoing continuing education programs and inservices.
28. Conducts staff inservices as requested.
29. Attends staff meetings as requested.
30. Fosters professional relationships as evidenced by educational programs and training.
31. Participates in performance improvement activities.
