Job Description


Inabia is seeking a motivated Physical Therapist to join the outpatient rehabilitation team  in Concord, California. The position is ideal for a clinician who enjoys hands-on patient care, strong interdisciplinary collaboration, and a consistent weekday schedule.


Key Responsibilities

  • Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
  • Provide daily treatments, track functional progress, and support discharge planning.
  • Collaborate closely with therapy, nursing, and interdisciplinary teams.
  • Complete accurate, timely, and compliant clinical documentation.
  • Manage a steady outpatient caseload while maintaining quality of care and patient satisfaction.
  • Support clinic flow and communicate effectively with patients, families, and referring providers.


Qualifications

  • Active California Physical Therapist license, or eligibility to practice in the state, required.
  • Outpatient rehabilitation experience preferred.
  • Strong communication, teamwork, and documentation skills.


Schedule

  • Shift: 5 x 8 Day shifts.
  • Typical schedule: M-F 8a-5p.
