Scope of Position

The Medication Management / Transitions of Care (TOC)Pharmacy Technician provides a valuable patient-centered service by focusing on facilitatingTOC,including obtaining and documentinga complete andaccurateadmission medication history and allergy information in the electronic medical record (EMR)as well asassisting with the discharge medication process. The TOC Pharmacy Technician completes the admission medication history during an inpatient admission, ambulatory encounter, pre-admission testing, surgery encounter, or within a procedural area under the supervision of a pharmacist, physician, or advance practice providerInformation can be gathered from a face-to-face or telehealth interview withthe patient chart and communication with patients, caregivers, physicians,retail pharmacies, and long-term care facilities The TOC Pharmacy Technician will make a good faith effort to ensure the documented information is complete and accurate by identifying and facilitating correction of medication discrepancies or escalating to a pharmacist or provider for follow up, if needed, thereby aiding in the hospital’s medication reconciliation effortsThroughout the hospital admission, the TOC Pharmacy Technician is responsible for collaborating with the patient and care team to facilitate discharge, which includes communicating options for discharge medications andlong-term medication delivery with the patient and/or caregivers, obtaining credit card information and facilitating Meds to Beds medication delivery, if applicable, and assisting with accessing coverage for medication,and engaging the appropriate staff such as the Medication Assistance Program if medication access needs are identified or case management if a prior authorization is required.

The TOC Pharmacy Technician promotes a culture of safety by adhering to all hospital and department rules, regulations and requirements including those outlined in the Medication List Documentation and Reconciliation policy, education videos, and Standard Operating Procedures.The TOC Pharmacy Technician must participate in continuing educational opportunities to meet the annual competency requirements and participate in continual training opportunities provided by the department to develop ongoing skills in other areas of pharmacy. The TOC Pharmacy Technician acts as a positive role model for all TOC pharmacy technicians and acts in an encouraging manner conducive to the hospital, department and administration’s visions and values.

The TOC Pharmacy Technicianis responsible for obtaining a complete and accurateadmissionor ambulatory medication history and allergy informationThe TOC Pharmacy Technician will help to streamline and facilitate the discharge medication delivery process when applicable.

Minimum Qualifications

High School graduate or equivalent.

Certified Pharmacy Technician.

2 years of relevant experience required. 4-6 years of relevant experience preferred.
