El Rio Community Health Center, based in Tucson, Arizona, opened its first clinic in 1970. Today, we have multiple health centers throughout the community, and our practice continues to grow. Essential Duties & Responsibilities
• Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.
• Deliver evidence based, patient centered drug therapy recommendations and collaborate with the interdisciplinary care team.
• Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.
• Identify, prevent, and resolve medication related problems; support patient safety, quality improvement, and clinical guideline initiatives.
• Serve as the primary clinical pharmacy resource for patients, providers, and staff.
• Apply 340B program knowledge to ensure compliant, affordable medication access and sound pharmacy operations.

To become a part of our dedicated team, you need the following qualifications:
• PharmD or equivalent doctoral degree – Required
• 3–5 years outpatient or retail pharmacy experience – Preferred
• Active Arizona Pharmacist License – Required upon hire
• BLS certification including in person skills component – Required upon hire
