Shift:

Rotating ShiftOrders input is timely and consistent with workload demands. All medications and intravenous solutions dispensed are checked for accuracy and completeness. All routinely-scheduled pharmaceuticals are available for administration at the scheduled time. All dispensed pharmaceuticals are accurately weighed, measured, admixed, and labeled properly. Monitors work progress of technicians and assesses and assigns work priorities. Trains pharmacy students, pharmacy interns, technicians, and peers in systems related to pharmacy department. Demonstrates working knowledge of phone, CRT, and printers. Monitors drug usage as it relates to available supply and ensures that adequate supplies are stocked/ordered. Communication, both oral and written, is specific and informative. Parenteral products are prepared using aseptic technique. All patient charges and credits are current and accurate. Problems and concerns are reported to Supervisor in a timely and constructive fashion.

PHARMACEUTICAL CARE

  • Physicians orders are evaluated accurately for input into the pharmacy computer system for all disease states and patient age groups.
  • Incompatibility data, drug interaction data, and drug allergy information is thoroughly reviewed for each order processed.
  • Recommendations for therapeutic substitutions or optimizing drug therapy are clear and concise.
  • Patient counseling and drug information is presented in a knowledgeable and informative manner.
  • Adverse drug reactions reported are accurate and thorough.
  • Pharmacokinetic recommendations for optimum dosage adjustments result from effective patient evaluation and physician consultation.
  • Parenteral nutrition management follows procedural guidelines to formulate nutritional assessment and treatment goals.
  • Monitors patient drug therapy with regard to drug use evaluation criteria.
  • Documents interventions in pharmacy computer system.
  • Documents events and near misses in event report form and sends to Pharmacy office for review.

AGE-RELATED PATIENT CARE

  • Demonstrates the ability to obtain information and interpret information in terms of the patient's needs.
  • Demonstrates appropriate knowledge of growth and development.
  • Demonstrates an understanding of the range of treatment needed by all patients including neonates, pediatrics, adult, and geriatric.
  • Demonstrates ability to assess and implement age-related dosing protocols.

COORDINATION

REGULATIONS, POLICIES, AND PROCEDURES

  • Nursing unit inspections are completed monthly.
  • Quality Control documentation and logs are complete with signatures.
  • All controlled substance documentation is legible and complete in accordance with hospital and government regulations.
  • Chemotherapy and investigational drug policy, procedure, and protocols are strictly adhered to.
  • Prescriptions are dispensed in accordance with State and Federal laws.
  • Departmental and hospital policies and procedures are reviewed annually to reinforce compliance and knowledge thereof.
  • Maintains current knowledge of pharmacy profession as well as applicable laws and regulations.

MINIMUM QUALIFICATIONS

  • Graduate School of Pharmacy with a minimum of a Bachelor's degree; Pharm D is preferred.
  • Previous experience as a Pharmacist in a hospital setting is preferred.

LICENSURE

  • Current Florida Pharmacy License
