This position serves as a clinical pharmacist caring for adult patients on hospitalist and cardiology services. The pharmacist works closely with physicians, advanced practice providers, nurses, care managers, and other members of the interdisciplinary healthcare team to optimize medication therapy and improve patient outcomes across a broad range of acute and chronic disease states. Responsibilities include supporting patients throughout their hospitalization, addressing medication-related barriers to care, and facilitating safe transitions between care settings.
A primary responsibility of this role is active participation in multidisciplinary rounds, providing evidence-based pharmacotherapy recommendations for complex medical patients with conditions such as diabetes, heart failure, infectious diseases, anticoagulation-related disorders, chronic kidney disease, substance use disorders, and other internal medicine diagnoses. The pharmacist is responsible for medication therapy management, antimicrobial stewardship, therapeutic drug monitoring, discharge medication review, patient education, medication safety initiatives, and ensuring adherence to best practices and clinical guidelines. The pharmacist also serves as a medication resource for providers, nurses, patients, and caregivers while helping to identify and resolve medication-related problems throughout the continuum of care.
The position offers numerous opportunities for professional growth and leadership through involvement in quality improvement initiatives, research projects, medication-use evaluations, transitions of care programs, and interdisciplinary committees aimed at advancing patient care. The pharmacist is also encouraged to support the educational mission of the department through the precepting and mentoring of pharmacy students, PGY1 pharmacy residents, and PGY2 Internal Medicine pharmacy residents.

This position is responsible for dispensing and compounding medication, counseling patients on medication and health subjects; interpreting prescriptions, managing patient drug therapy for complications, and performing drug utilization. The pharmacist collaborates with physicians, nurses, and other health-care practitioners to provide safe, high quality, and cost-effective drug therapy. Pharmacists will be required to serve as preceptors for pharmacy students and pharmacy residents in their area of training and expertise.

MINIMUM QUALIFICATIONS AND SPECIALIZED KNOWLEDGE

- Bachelor of Science or Doctor of Pharmacy degree from an accredited institution
- Current Ohio pharmacist license
- Previous pharmacy experience
- Basic Life Support Certification—current or obtained during the orientation period (unless greater than 50% of scheduled work hours, as determined by the manager, are completed off-site or not in a patient care setting)
- Completion of a Pharmacy Residency (PGY1) or equivalent hospital experience required. Completion of a Specialty Pharmacy Practice Residency (PGY2) or equivalent experience preferred for certain areas to staff.
- Pharmacy Board Certifications or other certifications
- Advanced-level degree in healthcare field
- Collaborative practice experience
- Advanced Cardiac Life Support Certification (may be required to obtain as determined by manager)
- Pharmacokinetic drug dosing experience
- Drug therapy management

50% - Reviews provider orders for correct drug choice, dosage form, dosing schedule, and possible drug-drug interactions or allergies. Contacts providers to clarify orders as appropriate. Dispenses and compounds medications and dispenses drug therapy-related devices in accordance with standards of practice and OhioHealth pharmacy policies. Ensures all medications, chemicals and supplies are dispensed without suboptimal integrity. Ensures appropriate labeling for proper administration of medication. Interprets prescriptions and determines compatibility in accordance with standards of practice. Prepares care plan for patients to ensure optimal therapeutic outcomes and maximize compliance with therapy instructions.

20% - Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions issued to patients in a consulting relationship with a provider. Provides drug information to providers, other health care professionals, and patients as needed.

10% - Counsels individuals regarding their drug therapy, including recommending drug therapy-related devices, assisting in the selection of non-prescription drugs for the treatment of common diseases and injuries, and providing instruction in their proper use.

10% - Provides associate and learner development and training, including precepting students and residents. Provides educational programs for pharmacy, nursing, and medical staff as needed.

10% - Directs and guides pharmacy technicians and pharmacy support personnel.

Ambulatory Care/Population Health

50% - Manages individuals’ drug therapy by initiating, monitoring, and modifying prescriptions in collaboration with a provider. Prepares individualized care plan to ensure optimal therapeutic outcomes, avoid adverse events, and maximize adherence with therapy instructions. Provides drug information to providers, other health care professionals, and patients as needed.

20% - Partners with individuals (patients, caregivers, and other health care professionals) to prevent, manage, and/or slow progression of disease through patient advocacy, wellness and health promotion, triage and referral, patient education, and self-management.

10% - Assist with guideline/policy development and implementation as well as participate in performance improvement initiatives, including those for practice advancement.

10% - Provides associate and learner development and training, including precepting students and residents. Provides educational programs for pharmacy, nursing, and medical staff as needed.

10% - Directs and guides pharmacy technicians and pharmacy support personnel.

Retail

55% - Leads safety, stewardship, and utilization strategies for designated site(s) in collaboration with system team. Facilitates and manages activities of various site and system committees and clinical guidance councils. Develops, enforces, and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient, and appropriate medication therapy use. Utilizing data, published best practice standards, and published events or recommendations, identifies opportunities for proactive change in the medication-use system to improve patient outcomes. Integrates medication improvement initiatives, activities, and changes with clinical and operational department programs and committees. Responsible for ensuring compliance with evidence-based recommendations, national patient safety recommendations, and regulatory standards related to safe and appropriate medication use.

OTHER INFORMATION

Schedule Mix of day and evening shift responsibility with every 4th weekend and 2 major holidays per year. Night shift rotation required to fill planned time off.

Progressive Practice Model: Mixed model where all pharmacists rotate through central pharmacy and 1-2 clinical areas. Clinical areas that pharmacists support include general medicine, infusion/oncology, operating room, cardiac care, critical care, emergency medicine, trauma, anticoagulation clinic, and more. Pharmacy services are well integrated into the interdisciplinary teams across the campus and organization.
