Job Description

Reports To : Director of Inpatient Pharmacy

FLSA Classification: Full-Time Salary Range $135,000 - $150,000

  • Responsible for the safe, efficacious, and cost-effective use of drugs through the application of specialized knowledge, skills, and professional judgment.
  • Reviews and assesses medication orders from authorized providers to identify, prevent, and resolve any potential and actual drug related issues.
  • Applies expert knowledge of drugs and related protocols to verify physician orders for medications.
  • Makes appropriate evidence-based, patient-centered medication recommendations.
  • Contacts nurses and physicians for collaboration and communicates effectively when necessary to verify orders and obtain additional information regarding the order to facilitate care.
  • Provides recommendations on appropriate therapies or alternatives when appropriate including, but not limited to pharmacokinetic dosing, renal/hepatic dosing, and antimicrobial stewardship.
  • Assists in monitoring all aspects of clinical/pharmacy services and medication therapies by reviewing patients’ profiles/charts to ensure safe, efficacious, and cost-effective drug therapy for all patients.
  • Responds to emergency situations.
  • Performs other operations/clinical functions or projects as assigned.
  • Responds to drug information requests and makes clinical recommendations to health professionals and patients when requested.
  • Actively engages in and contributes to the effective coordination of pharmacy operations in a critical access hospital.
  • Maintains proficiency in hospital computer system and participates in the daily operations of the pharmacy and medication dispensing.
  • Oversees pharmacy operations and supervises technician activities in the absence of the Director of Pharmacy to ensure compliance and to optimize efficiency and productivity within the pharmacy department.
  • Maintains security of pharmacy and controlled substances
  • Manages controlled substances and inventory control and compliance including regular documentation and completion of checklists.
  • Completes monthly unit inspections throughout facility.
  • Assists the director in compliant and efficient operations of the pharmacy and manages regulatory tasks related to maintaining the pharmacy’s accreditation, narcotic control and inventory management.
  • Assists the pharmacy director in implementing operational changes within the pharmacy such as the use of new systems or adjustments of standard procedures.
  • Other duties as assigned.

Education and Experience

  • Graduate of an ACPE accredited College of Pharmacy
  • Doctor of Pharmacy degree (PharmD) preferred
  • Current licensure by the CO Board of Pharmacy as a registered pharmacist
  • Hospital pharmacy experience highly preferred

About Pioneers Medical Center

Pioneers' history began in 1947 when Freeman Fairfield, a previous resident of this community, approached the County Commissioners about establishing a hospital. Over the next several years, Mr. Fairfield worked with the Commissioners and a group of local citizens to build a hospital for the Meeker community. Opening its doors in 1950, Pioneers began offering acute health care and surgical services to the community, licensed through the State of Colorado Department of Health and Human Services. We are Medicare and Medicaid-certified, accept private insurance, and, of course, self-pay. Financial arrangements may be set up through the Business Office.

In 1964, the Walbridge Memorial Convalescent Wing opened its doors for long-term care, made possible through the generosity of the wills of Carl and Adelaide Walbridge. Attached to the north side of the hospital, the Wing, with its 33 beds, brings skilled and intermediate nursing to residents who are unable to care for themselves. Through the Wing's rehabilitation services, exercise, and activity groups, each resident's care is individualized to meet his or her physical, emotional, nutritional, and spiritual needs. The Wing is licensed by the State of Colorado Department of Health and Human Services, and accepts Medicare, Medicaid, and private-pay residents.

First-line health care services are delivered through Meeker Family Health Center. The MFHC originated in 1990 as a rural health clinic, and its physicians, nurse practitioners, and staff continue to strive to meet our community's and visitors' healthcare needs.
