Job Description

INFECTIOUS DISEASE PHARMACIST

  • Work collaboratively with a multi-disciplinary team to promote the optimal use of antimicrobial agents, reduce the transmission of infection, and educate health professionals, patients, and the public.
  • Provide direct patient care, including treatment assessment and medication management activities.
  • Clinically evaluate all patients in a specified area on a daily basis with relevant documentation placed in the medical record regarding pharmacy recommendations, plans, interventions, and medication reconciliation.
    Pharmacists help to ensure that the prophylactic, empirical, and therapeutic uses of antimicrobial agents result in optimal patient outcomes.
  • Pharmacists provide recommendations to the Pharmacy & Therapeutic Committee to ensure that the number and types of antimicrobial agents available are appropriate for the patient populations served; analyzes data to ensure patient outcome data is used to assess the effectiveness of antimicrobial-use policies throughout the organization; works with the laboratory, infectious disease specialists, and infection in compiling susceptibility reports (at least annually) for distribution to prescribers to guide empirical therapy; utilizes information technology to enhance antimicrobial stewardship through surveillance, utilization and outcome reporting, and the development of clinical decision-support tools; and reduce potential errors and adverse drug events.
  • Pharmacists detect and prevent antibiotic-related drug-drug interactions; monitor time-sensitive automatic stop orders; perform automatic changes from intravenous to oral antibiotic therapy; dose adjust antibiotics in cases of organ dysfunction; optimize therapy for highly drug-resistant bacteria; achieve central nervous system penetration where needed; intervene where therapy might be unnecessarily duplicative; review microbiology cultures to target susceptible therapy of the infecting organism or stop antibiotics when culture results suggest infection unlikely.
  • The primary mission of the Infectious Disease Clinical Pharmacist is to improve patient safety.
  • Provide cost-effective management of drug utilization within the clinic.
  • Monitor process and outcome of patient care relating to drug therapy.
  • Monitor polypharmacy and high dose pharmaceutical dosing to prevent adverse effects.

Requirements

Job Requirements:

Minimum of one year of pharmacy experience. Previous inpatient experience is preferred. Board Certification Preferred but not required.

Bachelor’s degree in pharmacy. Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).

Licensure/Registration: Current, valid, full and unrestricted license in one of the 50 states and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the United States.

Certification-BLS American Heart Association

Advanced Cardiac Life Support (ACLS) – Emergency Department Pharmacists only

Or

Degree-Doctor of Pharmacy degree (Pharm.D.)

INFECTIOUS DISEASE PHARMACIST - A minimum of one-year pharmacy experience in Infectious Disease within the last 3 years. Completion of an ASHP accredited PGY-1 Pharmacy Practice Residency is required. ASHP accredited PGY-2 Specialty Residency in Infectious Disease or Infectious Disease Pharmacy Fellowship or equivalent experience required. Board certification is preferred. Chief, Department of Pharmacy has to waive requirements.
