About the Job
Responsible for coordinating and monitoring pharmacy services for home care and ambulatory patients in accordance with CHOP Home Care, CHOP policy and all regulatory and accrediting agency guidelines

What you will do

  • Responsible to provide accurate and timely infusion services for Home Care and Ambulatory patients.
  • Provides clinical practice/monitoring based on CHOP Home Care, CHOP policy and all accrediting agency and regulatory guidelines for home infusion, pharmacy dispensing, and ambulatory patients.
  • Provides quality pharmacy and home infusion services and collaborates with physicians, nurses, other health care professionals as well as the patient/caregiver to deliver safe patient care and optimal patient care and outcomes.
  • Responsible to complete all patient documentation timely and in compliance with CHOP Home Care policy.
  • Apply knowledge of USP 797 Guidelines & State Pharmacy Regulations
  • Provides and receives accurate and timely communication to render interdisciplinary patient care.
  • Provides drug information and/or presents educational programs to healthcare professionals and patients.
  • Provide training and orientation to new home care staff regarding pharmacy services.
  • Supervise pharmacy technicians and interns to ensure that pharmacy products are prepared and distributed accurately, with the appropriate quantity and at the time the product is needed.
  • Participates in departmental process improvement initiatives.
  • Performs other duties as assigned by Home Care Management


Education

  • Bachelor's Degree Pharmacy - Required


Experience

  • At least one (1) year previous experience - Required
  • Previous experience with IV medications - Required
  • At least two (2) years previous experience - Preferred
  • Home Care or Hospital Pharmacy - Preferred
  • Pediatric experience - Preferred


Knowledge, Skills and Abilities

  • Strong interpersonal, communication and organizational skills.

SALARY RANGE:

$56.70 - $75.10 Hourly

Salary ranges are shown for full-time jobs. If you're working part-time, your pay will be adjusted accordingly.
