Job Category: Allied HealthIn an environment of continuous quality improvement, the Pharmacist Clinical Coordinator is responsible for developing, implementing, and overseeing clinical pharmacy services to ensure safe, effective, and evidence based medication use. This role serves as a clinical leader and resource to pharmacists, technicians, nurses, and providers while supporting quality improvement, regulatory compliance, and optimal patient outcomes. In the absence of the Pharmacy Director, the Pharmacist Clinical Coordinator may be required to perform the essential duties of the Pharmacy Director. Exhibits the MHS Standards of Excellence and always exercises strict confidentiality.
Job Functions:
- Lead clinical pharmacy programs with primary accountability for Pharmacy & Therapeutics (P&T) Committee operations, pain stewardship, and active participation in antimicrobial stewardship activities in collaboration with the Antimicrobial Stewardship Pharmacist.
- Assess, develop, coordinate, implement, and maintain drug policies and clinical pharmacy programs that support safe, effective, and optimal pharmaceutical care.
- Collaborate with the Director of Pharmacy to develop and implement the Annual Clinical Plan.
- Provide leadership in the development, review, and maintenance of drug policies that promote safe, effective, and cost-efficient medication use.
- Develop, implement, and maintain clinically relevant tools that support clinical pharmacy programs and assist with pharmacy operations; complete required MUEs annually.
- Review and evaluate medication orders and patient care plans to ensure compliance with current clinical guidelines, including appropriate drug selection, dosing, frequency, interactions, and routes of administration.
- Collaborate with multidisciplinary teams to optimize patient outcomes, provide evidence-based therapeutic recommendations to providers, and remain current with primary literature and national guidelines.
- Provide ongoing clinical education to pharmacists, technicians, nurses, and providers.
- Participate in medication profile review, pharmacokinetic and drug therapy consults, renal dose adjustments, and patient education as needed; monitor drug therapy, laboratory data, and drug levels and intervene appropriately.
- Assists with staffing shifts including weekends during times of critical staffing needs
- Assumes all other duties and responsibilities as requested by the Director of Pharmacy.
QualificationsMinimum Education/Experience Required:
- Current licensure in the State of Ohio as a pharmacist required.
- 3 years of inpatient Hospital pharmacy experience preferred.
Special Knowledge, Skills, Training:
- Verbal and written communication skills are essential, manual dexterity and near visual acuity.
Compensation Details: Education, experience, and tenure may be considered along with internal equity when job offers are extended.
