Pharmacist-Ambulatory (Heart & Valve Center)

Full Time I First Shift

Hospital Pharmacy I Lexington, KY


Department of Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.

  • An Ambulatory Pharmacist Heart & Valve Center is responsible for providing clinical pharmacy services for ambulatory patients through accurate medication reconciliation; developing patient care plans including the selection, dosing, and monitoring of drug therapy; promoting medication adherence; and educating patients and other health care providers regarding complexities of drug therapy.
  • Pharmacists will work under a collaborative care agreement with clinic providers; using approved protocols for disease states such as heart failure, hypertension and etc.
  • Partners with providers and clinical staff on integration of the pharmacy team to coordinate care with patients.
  • Communicating with physicians and other clinicians as necessary to facilitate exemplary service for new and existing clinic patients.
  • Maintains onsite clinic pharmacy services, operational policies and procedures, clinical pharmacy workflows, and practice expectations.
  • Participates actively in multidisciplinary committees, teams, working groups, and/or task forces related to the care of patients as the pharmacy representative.
  • Educate clinic teams, pharmacy teams, and other professionals through a variety of educational venues including grand rounds and/or focused in-services.
  • Addresses inquiries from clinic staff, providers, patients, and families.
  • Ensures all state and federal regulatory requirements and accreditation standards are met and appropriately documented

Requirements:

  • Bachelor’s degree Pharmacy.
  • One year of experience.
  • Kentucky Board of Pharmacy License in good standing as applicable to practice site.
