At St. Anne’s Place, we provide specialized assisted living support tailored to the unique needs of vulnerable older adults with complex requirements. Our personalized services ensure that each resident can live safely and healthily within the community as they age. Our primary aim is to delay or even eliminate the need for institutional care for as long as possible. Our program provides 24/7 care through our dedicated Personal Support Workers team. We offer a range of services including personal care assistance, essential housekeeping, laundry services, medication support, and appointment escorts. Our in-house Psychogeriatric Case Managers deliver personalized one-on-one support, care coordination, crisis intervention, system navigation, and client advocacy. They connect clients with the necessary services to help them become healthier, happier, and as independent as possible. St. Anne’s offers a comprehensive meal service, with a delicious main meal served daily at noon, and breakfast and supper available for purchase. Residents can choose to opt into the full meal program or select individual meals.
Additionally, St. Anne’s Life Enrichment program is designed to enhance mental, physical, and spiritual health through social interaction and a sense of belonging within a welcoming and supportive community. This program provides residents with numerous opportunities to enjoy life, try new activities, and become part of a vibrant community. From summer BBQs to holiday celebrations, we strive to cater to the interests of our residents. St. Anne’s Place is a 112-unit high-rise located on Dufferin Street near Dundas Street, providing affordable housing through self-contained studio and one-bedroom apartments. The top floor boasts a bright solarium and a dining room with panoramic city views. On the main floor, you’ll find a cozy lounge area perfect for meeting friends in the café space. Additionally, there is an on-site doctor’s office and a convenient laundromat. Enjoy the convenience of numerous shops and restaurants just a short stroll away. St. Anne’s Place is easily accessible via TTC, with Dufferin Mall and Trinity Bellwood’s Park only a quick ride away. The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support, as assigned, the needs of the clients with mental health and/or addictions challenges. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.
