Perinatal Case Manager (CPSP Program)
General Purpose
Under the direction of the Chief Clinical Services Officer, the Perinatal Case Manager coordinates patient care in accordance with the Medi-Cal CPSP program. This position provides nutrition, health education, and psychosocial assessments to support healthy pregnancies and positive birth outcomes.
Key Responsibilities
- Conduct initial, trimester, and postpartum assessments per CPSP guidelines
- Develop and monitor individualized care plans
- Coordinate medical appointments, support services, and referrals
- Provide prenatal, childbirth, and breastfeeding education
- Serve as liaison between patients and the care team
- Maintain accurate documentation, billing, reports, and compliance records
- Participate in quality assurance and outreach activities
- Perform basic clinical tasks per protocol (e.g., vital signs)
- Other duties as assigned

Qualifications
- Experience in perinatal services, maternal-child health, or case management preferred
- Knowledge of CPSP/Medi-Cal guidelines preferred
- Strong organizational, communication, and documentation skills
- Ability to work effectively in a team-based clinical environment
