What you will do:

The LMSW/LCSW Social Work Case Manager provides advanced clinical social work services, care planning, case management, and community resource coordination to ALS patients and their families who utilize the services of ALS Regional Center. Work involves the use of sophisticated interviewing, assessment, and multi-modality psychotherapeutic interventions aimed at positively affecting the physical, cognitive, and emotional functioning of patients and their families. Work involves use of high-level creative problem solving and advanced communication skills while performing under mainly autonomous conditions. The master's level social work case manager identifies barriers which impact optimal patient functioning and ensures patient receives appropriate resources in most appropriate setting. Also actively participate in QI initiatives within the hospital and department. Supervision is available as needed. The MSW Social Worker is available as an advanced clinician to consult with the multidisciplinary ALS Team.

Responsibilities

  • Screens and accepts appropriate referrals.
  • Incorporates medical, psychological, psychiatric, psychodynamic, interpersonal/social, economic, cultural, emotional, and systemic factors in assessment.
  • Takes into account individual patient's special or age-related needs; understands growth pattern and developmental issues.
  • Identifies patient's strengths and resources.
  • Interviews patient/family and completes a thorough psychosocial history.
  • Identifies patient's support systems.
  • Conducts home visits when appropriate.
  • Identifies patient's wishes regarding care and advanced directives.
  • Incorporates DSM-5 diagnoses and utilizes personality inventory instruments when appropriate.
  • Structures family meetings for assessment purposes.
  • Documents assessment consistent with department standards.
  • Develops, implements, evaluates and revises, as necessary, a plan of care, including referrals to other healthcare and community organizations based on needs assessment.
  • Performs Cognitive Assessments and depression scales as appropriate, reporting results to ALS interdisciplinary team.

What you will need:

  • Master's degree in Social Work is required.
  • NYS LMSW or LCSW licensure is preferred.
  • Minimum of 1 year of experience in Medical, Psychiatric, or Health Care setting is preferred.

Pay Range: $30.00 - $43.08

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
