Position Details:
Responsibilities:
·Conducting psychosocial assessments related to the patient's illness and environment and effectively communicating findings to the registered nurse.
·Performing social evaluations and developing appropriate intervention plans based on assessment findings.
·Maintaining accurate and up-to-date clinical records for all patients referred to social work.
·Providing essential information and referral services to patients, families, and caregivers regarding practical and environmental needs.
·Acting as a liaison between patients, families, caregivers, and community agencies.
·Building and maintaining collaborative relationships with other healthcare professionals to support comprehensive patient care.
·Working with public and private agencies to leverage available resources for the benefit of our patients and our organization.
·Participating in the development of comprehensive care plans and engaging in case conferences as required.
·Assisting physicians and the multidisciplinary team in understanding the social and emotional factors influencing health problems.
·Contributing to discharge planning and ensuring a smooth transition for patients and their families.
Qualifications:
·LICSW (Licensed Independent Clinical Social Worker) required.
·Sensitivity and understanding of end-of-life issues.
·Expertise in psychosocial assessment and treatment.
·A collaborative approach to working with a multidisciplinary team.
·At least one (1) year of experience in Hospice care preferred.
·Strong verbal and written communication skills, as well as good organizational skills.
·Maintain current BLS CPR certification.
·Possess a valid driver's license and own a reliable, insured vehicle.
Benefits & Compensation:
· Rates: $60 per visit + mileage reimbursement.
