Job Location: Main Office - Newton, MA 02459Per Diem Hospice Nurse
Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last three years, our community based, mission-driven hospice and palliative care organization seeks a Per diem Registered Nurse to join its expanding clinical team. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest

Job Responsibilities:
- Participate as a member of the interdisciplinary team by providing clinical assessments of patients’ ADLs, and patient’s functional, psychological, and environmental needs.  Implements and monitors aspects of patient care including managing discomfort and symptom relief.
- Utilizes specialized nursing skills and agency resources related to end-of-life care in accordance with the Nursing Care Coordinator’s plan of care and in accord with Good Shepherd’s policies and quality assurance guidelines.
- Consult with and educate patients and families about disease process, self-care techniques, end-of-life, and the processes for dealing with issues of ethical concern.
- Consult, collaborate, and coordinate patient care with various health care facilities and professional health care providers.  Inform physicians and other personnel of changes in patient’s needs and outcomes of interventions.

Education, Skills and

Experience:
- Licensed Registered Nurse in the State of Massachusetts
- Sensitivity and understanding of end-of-life issues
- Experience in community health, hospice, oncology or related field preferred.
- Must have demonstrated skills in assessment, pain management, and symptom control
- Excellent organizational and time management skills
- Exceptional communication skills
