Job Description

By The Bay Health, a non-profit established in 1975, set the standard for hospice in the U.S. by emphasizing the role of the patient in making important medical decisions. Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

The Hospice Physical Therapist (PT) provides skilled physical therapy services to patients in their homes, helping them maintain independence, maintain safe mobility, and enhance quality of life. The PT develops individualized treatment plans, delivers evidence-based interventions, and collaborates with an interdisciplinary team to ensure coordinated, patient-centered care.

Schedule:

Per diem; 2 days a week or 6 shifts per month; 8:30am - 5:00pm

Location:

Marin County with coverage to Sonoma and S.F. Counties as needed

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities include (but are not limited to):

Patient Evaluation & Care Planning

  • Conducts comprehensive assessments of patient mobility, strength, balance, coordination, pain, and functional status.
  • Evaluates home environments for safety and recommends adaptive equipment or modifications as needed.

Treatment Implementation

  • Instructs patients, families, and caregivers on safe exercise techniques, use of assistive devices, and energy conservation.
  • Modifies treatment as necessary to maximize progress and safety.

Communication & Coordination

  • Collaborates with the RN Case Manager, , medical social work, and home health aides, and other members of the team as indicated.
  • Communicates changes in patient condition promptly to the physician and care team.

Documentation & Compliance

  • Completes all documentation timely and accurately within the EMR system.
  • Ensures all visit notes and progress reports comply with Medicare, CMS, and agency requirements.

Professional Accountability

  • Maintains current knowledge of physical therapy techniques, hospice regulations, and infection control practices.
  • Participates in continuing education, in-service programs, and quality improvement activities.

Education/Experience

  • Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s or Doctor of Physical Therapy)
  • Current, active Physical Therapy license in the State of California
  • Minimum of one (1) year clinical experience in physical therapy; home health or hospice experience preferred

Certificates, Licenses, Registrations:

  • Current California Physical Therapy license
  • Current CPR certification (BLS for Healthcare Providers)

Pay Range:
$69.00 - $76.60 hourly
