Per Diem Direct Care Supervisor

Department: Memory Support

Employment Type: Flexible

Location: Chelsea, MI

Schedule: Flexible-Per Diem |Work a minimum of 4 shifts per month, based on needs of the department. Two (2) shifts must be on a weekend.
Shift Premiums: 2:30 pm - 10:30 pm $ 1.90 | 10:31 pm - 7:00 am $ 1.35 | Weekend (Fri 10:31 pm - Sun 11:00 pm) $ 1.00

What You’ll Do:
Join our team as a Direct Care Supervisor, where you'll guide and support caregivers, ensuring our residents receive the compassionate care they deserve. In this role, you’ll be a key resource for your team, providing mentorship, problem-solving, and fostering a positive, team-oriented environment.

Key Responsibilities

  • Lead and Support – Supervise caregivers in performing daily living activities based on individualized service plans (ISPs).
  • Maintain Smooth Operations – Ensure proper staffing levels and collaborate with providers.
  • Coordinate Medication and Care – Process medication orders and oversee resident assessments during intake and admission.
  • Coach and Mentor – Provide hands-on training, feedback, and guidance to caregivers, helping them grow and excel.
  • Monitor Safety and Compliance – Review incident reports and ensure policies and procedures are followed.
  • Provide Hands-On Care – Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
  • Support Mealtime– Help residents enjoy their meals and stay hydrated for good health.
  • Monitor with Watchful Eyes – Report any changes in conditions to the leadership team ASAP.
  • Demonstrate Respect & Professionalism – Maintain confidentiality while delivering top-notch care.

Skills, Knowledge & Expertise

Education Matters!
  • A high school diploma or equivalent is a must.
  • Got training as a Medical Assistant, or Certified Nurse Aide (CNA)? Perfect!
Experience That Counts!
  • If you have experience in medical assisting, EMS, or caregiving—awesome! We’d love to hear about it.
  • Bonus points for leadership or supervisory experience!
Skills That Make a Difference!
  • You should know your way around medical terminology, medications, and body structure.
  • Hands-on experience with injections and wound care? Even better (don’t worry, an RN will evaluate your skills).
Certifications & Physical Abilities!
  • CPR certified? Great! If not, no worries—you’ll need to get it within 90 days.
  • Need to lift up to 35 lbs.? You got this! Heavier tasks? We’ve got the right equipment and team support
