Job Description

The Clinical Program Coordinator assesses, plans, implements, and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

2 part time positions, 20 hours a week, day shift (Monday - Friday 0730-17:00)

No nights/weekends/holidays. One day per month we work 7:30PM-9:30PM to prep for conference with the provider on Teams.

Essential Functions

  • Provides patients with information regarding available resources and options to facilitate shared decision making with their care providers
  • Provides patient and community education
  • Ensures optimal patient care using evidence-based, clinical best practice models, appropriate staffing and technologies and continuous process improvement measures
  • Involved in patient engagement efforts and monitors patient perception of the quality of care delivered
  • Coordinates care across the continuum, including planning and implementing clinical protocols and practice management guidelines, monitoring care, and serving as an expert resource for clinical practice.
  • Monitors clinical outcomes and system issues related to quality of care
  • Stabilizes the complex network of many disciplines that work in concert to provide high quality care, serving as an internal resource for staff in all departments

Required Qualifications

  • Current RN licensure in the state of practice. - and - RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.
  • Basic Life Support Certification (BLS) for healthcare providers.

Preferred Qualifications

  • Leadership and mentoring experience
  • Two years of clinical nursing experience
  • Pediatric RN work experience in OR or NTU at Primary Children's Hospital

Physical Requirements

  • Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.
  • Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
  • Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$41.20 - $62.17
