Registered Nurse (RN) Care Manager – Pediatric Home Care

Affirmed Home Care is seeking an experienced, compassionate, and highly motivated Registered Nurse (RN) Care Manager to support pediatric clients throughout Westchester County, NY.

This per diem, field-based opportunity is ideal for an independent nurse who enjoys flexibility, autonomy, and building meaningful relationships with families while delivering exceptional one-on-one care in the home setting.

RN Care Manager Responsibilities

• Deliver high-quality in-home nursing assessments and case management services
• Develop, implement, and oversee individualized pediatric care plans
• Obtain and maintain physician orders in compliance with NYS regulations
• Monitor, document, and communicate changes in client condition
• Conduct supervisory visits and support home care staff in the field
• Ensure continuity of care while promoting client comfort, safety, and independence
• Collaborate with families, physicians, and interdisciplinary care teams

Qualifications

• Minimum of 2 years recent RN experience in pediatric home care or acute care
• Strong pediatric clinical skills and experience with:
  • Ventilator and tracheostomy care
  • G-tube/PEG tube care
  • IV infusions
  • Wound care management
• Active New York State RN License
• Current BLS Certification
• Recent physical exam (within 1 year)
• Current PPD/QuantiFERON (within 1 year) or chest X-ray (within 5 years)
• Valid MMR lab report
• Valid driver’s license and reliable transportation
• Authorized to work in the United States

Compensation & Benefits

Competitive Pay Rates

• $150 per assessment
• $125 per aide supervision visit
• $55/hour
