Licensed Practical Nurse?(?LPN?)?Responsibilities

1. Licensed Practical Nurse?( LPN?) provides direct patient care as defined in-the State-Nurse Practice Act.?
   
2. LPN implements-a plan of care-initiated-by the-clinician.?
   
3. Provides-accurate-and-timely-documentation consistent with the plan of care.?
   
4. Licensed Practical Nurse?( LPN?) assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.?
   
5. LPN-participates-in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the clinician and/or Clinical Supervisor.?
   
6. LPN uses equipment and supplies effectively and efficiently.?
   
7. Licensed Practical Nurse?( LPN?)?participates-in personal and professional growth and development.?
   
8. Performs other duties as assigned by the Registered nurse.?
   

Licensed Practical Nurse?(?LPN?)?qualifications and-skills

• Currently licensed-as a-Licensed-Practical-Nurse in Pennsylvania?
• Valid-CPR Certificate.?
  
• At least two years experience working with pediatric patients.?
  
• Compliant-with-accepted professional standards and-practices.?
  
• Demonstrates good verbal and written communication, and-organizational-skills.?
