Job Description

Cobble Hill is currently seeking a compassionate and detail-oriented Pediatric Intake Specialist to join our growing team.

Your Choice At Home, an affiliate of Cobble Hill Life Center, is a Certified Home Health Agency (CHHA) dedicated to providing culturally sensitive, family-centered care. Operating 24/7, we provide dignified in-home services with a special focus on supporting children and their families through high-quality health and personal care services.

The Pediatric Intake Specialist plays a critical role in supporting children and families as they begin home care services. This position is responsible for receiving and evaluating referrals, educating families on available pediatric services, coordinating admissions, and maintaining clear communication with referral sources and internal clinical teams. The Intake Specialist helps ensure eligibility, regulatory compliance, and a positive experience throughout the intake process.

Key Responsibilities of Pediatric Intake & Care Coordination:

  • Receive, review, and process pediatric referrals from hospitals, rehabilitation facilities, MLTCs, social workers, case managers, and community partners.
  • Communicate with parents, guardians, and caregivers to explain services and assess pediatric home care needs.
  • Screen pediatric patients for eligibility under the CHHA program.
  • Coordinate with referral sources to support a smooth transition from referral to admission.
  • Maintain consistent communication with families, caregivers, and referral partners from admission through discharge.
  • Accurately enter patient demographics and referral data into the patient care system.
  • Schedule Start of Care (SOC) visits for Registered Nurses.
  • Communicate with HMO families regarding authorization requests and provide timely updates.

Administrative & Family-Focused Support

  • Respond to phone and email inquiries from families, referral sources, and care coordinators with professionalism and empathy.
  • Address family concerns or questions promptly and respectfully.
  • Prepare intake packets and maintain organized pediatric patient charts.
  • File and manage active and discharged patient records in compliance with confidentiality standards.
  • Participate in performance improvement and quality initiatives.
  • Complete assigned tasks within established timeframes.
  • Foster a positive, supportive, and family-centered work environment.
  • Perform additional duties as assigned by the Operations Manager.

Requirements & Qualifications

  • High school diploma required.
  • Minimum of one (1) year of healthcare intake experience; pediatric experience strongly required.
  • Knowledge of CHHA program requirements.
  • Strong communication and interpersonal skills, particularly when working with families and caregivers.
  • Proficiency in computerized office systems.
  • Ability to collaborate effectively with families, clinicians, providers, and community partners.
  • Demonstrated professionalism, discretion, and ability to manage confidential information.
  • Strong organizational, problem-solving, and decision-making skills.
  • Ability to work both independently and as part of a team.
  • Understanding of cultural sensitivity and family-centered customer service.
  • Willingness to seek guidance when needed.
  • Must meet New York State Department of Health regulatory health requirements.

Physical & Work Environment

  • Ability to sit for extended periods.
  • Ability to lift, carry, and transport materials as needed during the workday.
  • Adequate vision and hearing, with or without reasonable accommodation, to perform essential job functions.
  • Work is primarily office-based with regular interaction with administrative staff, clinical teams, families, and caregivers.
