The Patient Services Coordinator (PSC) is the backbone of our care coordination. You will ensure patient visits are accurately scheduled and maintain constant communication with field staff to provide seamless continuity of care. This role is perfect for a multitasking professional who thrives in a fast-paced environment and enjoys logistics.
Key Responsibilities
• Schedule Management: Coordinate and edit daily, weekly, and weekend schedules for clinicians; manage reassignments for Admissions, Recertifications, and Resumptions.
• Staffing & Coverage: Ensure adequate coverage for vacations, holidays, and sick leave; monitor clinician productivity and per-diem utilization.
• Problem Solving: Resolve routing issues and schedule training visits for new clinicians; triage after-hours care coordination issues reported by On-Call teams.
• Reporting & Compliance: Maintain 100% accuracy in patient details, run daily reports to correct errors, and ensure all visits comply with insurance authorizations and physician orders.
• Administrative Support: Assist with office management tasks and internal patient transfers between branch offices as directed.
Qualifications
• Education: Bachelor’s degree or equivalent professional experience.
• Tech Skills: Advanced proficiency in Microsoft Office (Excel, Outlook, Teams, and Word).
• Communication: Exceptional interpersonal skills with the ability to handle sensitive situations professionally.
• Logistics: Strong organizational skills with a proven ability to multitask in a high-volume setting.
Preferred Experience
• Industry: 1+ year in a Home Health or healthcare environment.
• Background: Education in Human Services, Social Work, Psychology, or Business.
Physical Requirements
• Primarily a seated office role involving frequent computer/phone use; must be able to lift up to 25 lbs and travel locally (30%) as needed.
