Patient Relationship Liaison

The Patient Relationship Liaison will facilitate care coordination from acute to ambulatory and is a valued member of the healthcare team. They collaborate with Physicians, Case Managers and other members of the healthcare team to facilitate coordination of care and follow-up appointments to the ambulatory setting. Collaborates with Case Managers for guidance with medically and/or psychosocially complex cases. Serves as a resource for care coordination. Maintains current knowledge of available community resources, post-acute care and ambulatory options.

Schedule: Monday - Friday | 8:00am - 4:30pm

Location: Presbyterian Medical Center (Reimbursed travel to Huntersville Medical Center twice a week)

What You'll Do

• Guide patients through the transition from inpatient care to outpatient treatment by coordinating follow-up services and addressing potential gaps in care.
• Utilize dynamic patient census and discharge reports to prioritize outreach efforts and support effective care management planning.
• Collaborate with patients, providers, and care teams to arrange primary and specialty care visits, promoting adherence to post-discharge recommendations.
• Enhance the patient discharge experience by ensuring follow-up appointments are scheduled before discharge, improving continuity of care and patient outcomes.

What We're Looking For

• Scheduling experience within EPIC, highly preferred.
• Education: High School Diploma or GED, required. 2 Year / Associate Degree, preferred.
• Experience: 2 years in Healthcare, preferred.
• Additional Skills (required): Some weekend and holiday work required.
