Full Time Patient Navigator, SOR 4 Grant
BAART Programs is looking for an adaptable and effective Patient Navigator, SOR 3 Hub and Spoke Grant, to work with external entities including but not limited to: jails, hospital EDs, OBOTs, residential programs, other locations identified in need of help with outreach and education, to help identify persons with OUD in these settings and discuss referrals to MAT treatment and develop realistic individualized action plans for patients. Support patient to access the highest level of care available.

Salary ranges from $30.00 to $32.89 an hour.
The salary of the candidate(s) selected for this role will be set based on a variety of factors, including but not limited to, experience, education, specialty and training.

Today, BAART Programs, a brand of BayMark Health Services, is a multi-site, multi-service organization providing opioid use disorder treatment and rehabilitation for patients across the country. We provide high-quality, patient-centered treatment for addiction to prescription medications and other opioids at our 36 clinics. We combine evidence-based, medication-assisted treatment with counseling and behavioral services. In addition, integrated primary medical care and mental health services are available in select clinics.
