Job Summary:
Responsible for managing patient awaiting hospital DC lists, following up every two days for discharge updates, documenting status in McKesson, ensuring timely collection and uploading of discharge summaries.
Responsibilities:
Daily Case Management:
• Review and manage a daily list of pending or active patients.
• Follow up with patients, social workers, case managers, or patients’ families every two days to gather information regarding discharge timelines.
• Documenting all patient status updates via email and in McKesson notes.
· Discharge Coordination:
• Ensure timely collection of discharge summaries for patients returning to service.
• Upload discharge summaries to the intake system promptly to facilitate a seamless start of care.
· Timely Communication:
• Maintain consistent communication regarding patient statuses, ensuring no gaps of more than two days in follow-up.
• Respond promptly to all wellness and patient events emails to ensure all inquiries are addressed in a timely manner.

All team members in the above positions are required to assist in filling in for absent employees and assist with training new employees within their specific area of expertise.
Qualifications:
◦ At least two (2) years’ experience as a medical assistant, medical coordinator, or medical office receptionist
◦ At least one year of previous health care experience working

Education:
◦ High Schol graduate or equivalent- two years of college preferred.
