We are seeking an experienced and dedicated Patient Care Manager to join our team in Norwood. This role is critical in ensuring the highest standards of patient care within a home health setting. As a leader in this field, you will drive initiatives to improve patient outcomes and ensure that every individual receives personalized and compassionate care. Key Responsibilities:
Manage and oversee patient care services, ensuring compliance with all regulatory standards.
Lead, mentor, and support nursing staff, fostering professional growth and enhanced patient outcomes.
