About the Role

Our client is seeking a dynamic, highly qualified, and strategic Director of Patient Care Services (DPCS) to lead agency-wide clinical operations across the San Francisco Bay Area.

The DPCS will provide clinical direction, oversee branch Clinical Managers, drive strict regulatory compliance, and elevate overall patient care outcomes. We are looking for an assertive RN leader with direct communication, strong analytical decision-making capabilities, and a proven track record of owning quality metrics.

Key Responsibilities

• Clinical Operations Leadership: Direct and oversee daily home health clinical operations to ensure safe, efficient, and high-quality patient care delivery.
• Team Management & Development: Supervise, recruit, mentor, and evaluate branch Clinical Managers and clinical supervisory staff.
• Regulatory Compliance & Standards: Ensure strict adherence to Medicare Conditions of Participation (CoPs), California state regulations (Title 22 / 22 CCR § 74703), ACHC accreditation standards, and agency policies.
• Quality Improvement (QAPI & OASIS): Design and execute the agency’s Quality Assessment and Performance Improvement (QAPI) program. Monitor OASIS documentation accuracy, rehospitalization rates, and patient satisfaction outcomes.
• Performance Metrics & Oversight: Analyze clinical data to set performance priorities, resolve operational bottlenecks, and drive measurable quality improvements.
• Strategic Alignment: Balance executive business goals, field-clinician support, and top-tier patient advocacy.

Requirements & Must-Haves

• Active Licensure: Active, unrestricted California Registered Nurse (RN) License.Title 22 Eligibility (22 CCR § 74703(b))
• Home Health Industry Expertise: Deep expertise in Medicare-certified Home Health clinical leadership, including OASIS documentation, PDGM guidelines, and CoPs.
• Supervisory Track Record: Demonstrated success managing branch Clinical Managers and owning agency-wide quality outcomes.
• Location & Availability: Based in or around the San Francisco Bay Area with full availability for onsite operational leadership.

Preferred Qualifications

• 5+ years of multisite or multibranch clinical leadership in Home Health.
• Proven history owning survey Plans of Correction (CDPH/CMS) and QAPI programs.
• Active CPR / BLS Certification.
