Patient Care Coordinator

What You’ll Do

- Partner closely with internal clinical and operations teams to ensure smooth coordination of patient care
- Review patient charts to confirm all required documentation is complete prior to nursing coordination
- Coordinate with contracted nursing agencies to schedule medication administration appointments
- Send referral documentation and confirm patient scheduling details with all parties involved
- Manage and maintain the nursing referral log on a daily basis
- Reconcile nursing notes and agency invoices, ensuring accuracy before submitting to finance
- Serve as a central point of communication between patients, providers, and nursing agencies
- Triage and escalate any nursing-related concerns to the appropriate internal teams
- Represent the organization professionally in all interactions
- Collaborate cross-functionally to resolve issues quickly and efficiently
- Report any compliance concerns or irregularities as needed
- Support additional projects and responsibilities as assigned

What You Bring

- At least 1 year of experience in a related healthcare or coordination role
- Strong organizational skills with the ability to manage multiple moving pieces accurately
- Comfort making decisions and prioritizing in a fast-paced environment
- Proficiency with spreadsheets, email, and general computer systems
- Working knowledge of pharmacy or healthcare applications preferred
- Familiarity with medical terminology
- Strong data entry and documentation accuracy
