Citizens Health is recruiting a Patient Care Coordinator/ Swing bed Coordinator to work full-time in our hospital.

Position Information: The Patient Care Coordinator/ Swing bed Coordinator serves as a liaison between patients, families, and healthcare providers, ensuring seamless communication and coordination of care services while adhering to the organization's mission of enhancing the lives of those we serve. This role involves direct patient interaction, maintaining compliance with healthcare regulations and standards, and collaborating with healthcare professionals and administrative staff to foster a positive and efficient work environment. The Coordinator addresses patient and family goals and supports the overall objectives of the organization.

Pay Range: dependent upon experience.

What you’ll do

  • Coordinate all aspects of the swing bed program, including admissions, discharges and patient assessments.
  • Ensure compliance with Medicare and other regulatory requirements
  • Verify insurance, swing bed benefit days, and obtain authorization as needed.
  • Arrange Transport between facilities as needed.
  • Assess patients physical, emotional, and discharge needs including mobility, safety, therapy, and equipment.
  • Arrange durable medical equipment, home health, or rehabilitation services as part of discharge.
  • Monitor and report on program utilization and quality metrics
  • Serve as the primary point of contact for our patients and families, addressing concerns and facilitating access to services.
  • Facilitate communication among physicians, nurses, social workers, and other healthcare professionals to ensure coordinated care
  • Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed
  • Educate referring facilities, community, patients and families about the swing bed program and its benefits
  • Build and maintain relationships with external healthcare providers and streamline patients transitions
  • Maintain accurate and up to date patient records in compliance with healthcare providers to streamline patient transitions.
  • Provide clear instruction on home care, follow up appointments and medication regiments with patient and families and identify and address any discharge barriers.

What you bring to the table:

  • High School Diploma
  • College degree in health care preferred
  • Prior experience preferred in the hospital setting with focus on swing bed, social work, discharge planning, and case management.
  • Experience in office productivity software (email, calendar, word processing, spreadsheets, presentation software, etc.).

Typical Working Conditions:

Works in well-lighted and well-ventilated office. Requires constant concentration, tolerance of low level noise and numerous interruptions. Must have good communication skills for interaction with all other hospital departments, patients and their families. This list is not exhaustive and may be supplemented.

Typical Physical Demands:

Extended periods of sitting to complete tasks. Walking to meet with and assist patients and families..
