Ascension at Home together with Compassus

The Patient Care Coordinator's primary function is to schedule patient services and coordinate with staff.

This position is based in office (not remote) and the work hours are Monday-Friday 8am-5pm.


Position Specific Responsibilities

  • Schedules and appropriately documents patient schedules in a timely manner.
  • Participates in coordinating care with management and patient interdisciplinary team.
  • Provides effective communication to patients, staff members, other health care professionals, and referral sources.
  • Responds to agency and patient needs in a professional and creative manner.
  • Performs on-call responsibilities as assigned.
  • Performs other duties as assigned.


Education and/or Experience

  • High school diploma or GED required.
  • Experience with patient scheduling highly preferred. Prior home health scheduling strongly preferred.
  • Prior customer service preferred.
