As a Patient Care Coordinator in Home Health, you will support daily clinical operations by coordinating patient visits, managing staffing schedules, and ensuring timely initiation of care. Your ability to prioritize, communicate clearly, and adapt to rapidly changing needs helps maintain operational excellence and a positive experience for both patients and care teams.

As a Patient Care Coordinator, you will:

  • Coordinate Patient Admissions & Scheduling – Manage and schedule admissions, resumptions of care, evaluations, and other key clinical visits—ensuring timely initiation of care and alignment with patient needs and clinician availability.
  • Support Intake & Care Transitions – Partner closely with the Intake and Clinical teams to ensure smooth handoffs, communicate capacity updates, escalate urgent needs, and support on-call coordination when appropriate.
  • Maintain Accurate Schedules & Staffing Coverage – Monitor and update clinician schedules in real time, proactively addressing sick calls, vacation requests, and unplanned gaps to maintain efficiency and continuity of care.
  • Communicate Changes & Identify Barriers – Record schedule adjustments and communicate updates promptly to all relevant team members, ensuring transparency and alignment across the team. Recognize potential staffing or scheduling challenges early and escalate concerns to leadership to support strong operational decision-making.

This job also requires:

  • Highschool Diploma or equivalent required; Associate’s or Bachelor's degree preferred.
  • Prior experience in Home Health, healthcare scheduling, or related field strongly preferred.
  • Proficiency with scheduling platforms (e.g., Homecare Homebase) and Microsoft Office Suite.
  • Strong organizational skills with the ability to prioritize, adapt, and work efficiently under time-sensitive conditions.
  • Effective verbal and written communication skills to collaborate across departments.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job duties.
  • Ability to read, write, and speak English to ensure effective communication with team members, clients, and partners.
