We’re hiring a Patient Care Coordinator for our Austin and Dallas offices– a dedicated and organized professional who ensures seamless coordination of patient care, supports clinical teams, and serves as a vital link between patients, families, and hospice leadership. This role is essential to maintaining quality care and compliance while fostering compassionate communication. Must live in one of the two listed territories: Austin or Dallas, this is not a remote position.
You lead with empathy, organization, and attention to detail. You thrive in a mission-driven environment and are passionate about supporting patients and families while ensuring clinical operations run smoothly.
This position will also assume Patient Care Coordinator (PCC) duties for the Austin and Dallas offices.
