This is a meaningful opportunity for a Patient Care Coordinator who wants to make a direct impact in home-based care. As Circadia's Patient Care Coordinator, you'll be the link between technology and patient care — traveling to patient homes across Orange County to install our remote monitoring devices, take patients' vitals, and facilitate initial telehealth visits with our Nurse Practitioners.
